Your First Visit

When to arrive:

• At least 15 minutes early to allow for time to check you in and make sure that all of the correct paper work has been filled out.

What to wear:

• Loose fitting athletic clothes.
• Wear clothing that easily exposes the body part(s) of concern.
• A change of shoes/slippers to wear inside the gym.

What to bring:

• If referred from another provider, a copy of your script if it hasn’t already been faxed over to our clinic.
• A photo ID and Insurance card.
• A list of current medications.
• To make the check-in process go more quickly, print and fill out the New Patient Intake Forms to bring to your first visit.

If you are coming to physical therapy because of a surgery:

• Bring any hand-outs or information already given to you from your doctor regarding your surgery.
• Bring any equipment or braces provided to you by your doctor’s office.
